use your card, you will be allowed five (5) pieces of materials.

Card

The child must be
6 years old.
(can write first & last name)

You need to show proof
of current address.

A parent of legal guardian
must sign for the child to accept responsibility. Proof of
guardianship is required.

The first time you

or Regional

Must provide all
the informatio as an adult or child's card application.

residence's first card is free.

$2.00 fee will be charged for a replacement card.

will be renewed by computer upon expiration date.
